Okay this has really had me wondering for a while, but for the hundreds of years that they hadn't met Ichigo they all looked the same as they ever had looked, but a few years after he lost his powers the majority of them had a massive change in outward appearance...

That's about the best explanation you're gonna get. That, or something along the lines of, "Gotta keep readers interested! Make the characters look different! Not too different, really exactly the same, but different!" By the way, that should have been read in the voice of J. Jonah Jameson.
It doesn't make sense that a person would stay the same age for forty or fifty years then change after a year and a half, but it also doesn't make sense that when people die, they go to Soul Society, but they can die in Soul Society. What do they go then?
Also, Arrancars are basically evolved Hollows, and when Hollows get killed by the Zanpakuto, they are cleansed, and the original soul (of the human that turned into a Hollow) goes to Soul Society, except if the person was evil, then they go to Hell. Right? So, did all the killed Arrancars go to Soul Society as human souls? Is there a human Nnoitora walking around Rukongai?

You said if the person is evil, they go to hell. Nnoitra was pretty much on the evil side to me.
OT: You know Ichigo initiated massive changes, did you? And the Shinigami did look the same all those years because they age very damn slow - Rukia is already way way old in terms of age. Like centuries. But Kubo is kind of inconsistent with age statements and flashbacks.
And take note of personality and attitude changes. In the manga Toshiro Hitsugaya became some sort of brooding nerd with a loss of hair wax and a shorter haircut. But give him some hair wax, let his hair grow and make him scowl and you'll see the original one. The other two captains looked the same except for their new haoris. Renji still looks like an ass and Baldy on the left sports the same sneer since Volume 18.
And Rukia only got promoted and got a short haircut.
Not much of a major change to me.
