I know not if this particular plot exists, but there are many a disturbing hentai flick where girls get killed [they make me feel sickly]. Bible Black is a good place to start, if you want to see disturbing shit, muder of cute, innocent girls amung them.

I can't really think of any, good or bad, Anime or Visual Novels where the female lead dies because she betrayed the main character. Usually, betrayal first, then fight, then sex, then happily ever after.
