As much as I like anime and manga, after a while I'm sure everyone notices how formulaic it becomes. In my case, it becomes extra formulaic as I am a fan of harem/ecchi/romance comedies. There are only so many moe types, and characters eventually boil down to a mix and match of different parts, a la Ai Kora.
And then there are the other various tropes that make for easy setting-creation. Lost memories, childhood friends (my two least favorite), a promise, love at first sight, etc.
I think the thing I'm slowly starting to hate the most (in shounen anime) is the "X is created/founded/in existence to combat Y". I can't really think of specific examples right now since I tend to not read them anymore, but I'm sure you're familiar with the whole "XX years ago monsters/demons/robots attacked earth. To defend themselves, the government created an agency/a group of warriors came together/those with special powers appeared called YY to fight them." description. To be fair, they don't always follow the same path, but the description of it is enough to negate points in my book.
And blah blah blah YOUTUBE.

Ahh. You're talking about cliches.
For me, it will be the bad endings of a "childhood friends" relationship in relation to the harem / multi-arc nature of an anime. Even though it happens in real life, they can at least let it happen in anime's, just once.
Siscon comes second Third is lolicon and pettanko.
The best example for this statement, for me, is Toradora, where [I forgot the boy's name] chose taigah (loli pettanko) over Minorin (childhood crush) or Ami. Also, in Amagami SS, Rihoko's arc (Rihoko is Junichi and Miya's childhood friend). Her arc is also bad in the OVA.

Well, school settings are not so bad, they're overused, but I don't find them annoying.
But things I can't stand are: idiot protagonists in ecchi/ harem series (regular everyday guys like in Omamori Himari or C3 were actually good for me because of avoiding this)
And also (flame shield ready lol) I can't stand violent tsunderes anymore, I mean, their picking on the protagonist or anyone else is most of the time (if not always) played for laughs, even when sometimes they do something actually not pretty funny.

I get tired of the scenes where the protagonist falls on a female character and ends up groping her by accident and she beats his ass, I hate when that happens.

I don't know, nothing really gets on my nerves. If I had to pick something, it would be main characters with no redeeming qualities.

I'd like to see more manga like Monthly 'Aikawa' The Chief Editor. That one has a very likable guy as the protagonist who actually has some good lines and acts like a real person. Same with the chief editor too, she's very realistic and focuses on her job like most people and has realistic emotions. Very good series, relatable and humorous at times, definitely one of my favorites. I just wish more series was like this.

I have to say that I originally got into anime because of how different it seemed compared to the "predictable" American television. It was only years later I realized what bored me are tropes that I had seen a hundred times before. But after a couple of years of anime, I now know all those tropes. So television is a dead medium for me. This video captures it very well. Similarly there is a website that catalogs all the tropes that appear in a specific show/movie, made me realize just how similar all the shows and movies I like really are.
