If you scroll down the page on the second link, you'll see a sketch with three characters on it. It was a dakimakura cover that was supposed to release over a year ago, when the first season of Boku Wa Tomodachi Ga Sukunai was still airing. It never made it out of the sketch phase, and it looked like it was never going to be completed because the artist moved onto other items. But since season 2 has started, it looked like she's going to complete it. Personally, it happens to be one of my favorite artists. Pretty much everything in this thread at the time was really, really looking forward to it.
The covers with inserts are specifically designed for them; they usually have pockets on the inside of the cover, whereas others don't.

And it's a funny point you make about the sizes; years ago 150cm x 50cm was the standards, while it was the 160 covers that were strange. I've seen an increasing number of 170 covers over the last year or two (mostly by Marks, Anko, and a few other circles) and I wonder when it will become the new standard. Of course, it's nice to be able to vary up the size of specific covers to fit with the character's height proportionally. Not all covers need be 150/160/170, but can be fit to what best suits the character; this has already happened a bit, with a few 180/190 covers.
