Someone's actual "sex" is biological. What their chromosomes are.

Gender is really more of a linguistic term, but in English there isn't really much of a gender system, so it's more correct to use it in relation to sexual identity, as Tegumi said.
The gender of the person in the image is almost certainly female.
The sex is probably female as well, or at least a male with a good bit of surgery done.
People seem generally uncomfortable with saying the word sex, though. Like it's a dirty word.
