Well in the case of johnnywazza I think they're using a LiteStep theme. LiteStep completely replaces your normal Explorer program with a different one so it's not really for novices.

For mine I just used WindowBlinds and went with a nifty see-thru skin.

Actually I ain't using somethin too hard to manage. Progs like Aston and Litestep are teh funk, but need even more time then I care to put aside :P. I'm just usin a bunch of small progs. For those who are interested:
-Windowsblinds for like the start bar and windows borders etc (can't tell in my screenie)
-ObjectDock for the bar at the bottom
-ObjectBar for the mac-like bar at the top
-Rainmeter for the clock and system data
-CursorXP for the custom cursor

Sounds like a lot, but actually doesn't drain much in the way of comp resources. All it does drain is time I coulda spent elsewhere :P.
