i dont know much myself.. except the fact that it formats the video that you are watching into a more clearer/better sounding 'high-definition' quality video.
try it out yourself! (lol its gonna take a little longer to buffer though.)

I believe there's also &fmt=6
Adding "&fmt=6" will give you the video in 448x336 as opposed to the normal 320x240, and "&fmt=18" will give you a video that's set up like an iPod's video. Some videos work better in "&fmt=6" than "&fmt=18", so I'd try out both if you're downloading it.
And for those of you who don't know how to do it, here's a brief video. In HD! =D

Looks and sounds better for what I want, though half the vids in my playlist have either been removed or aren't ok for americans to watch. So damn sad...

I read somewhere that the sound is more distorted when using the secret code.. it's only the video that is improved. So I leave it as is for music.
It depends on the video in question. Usually, the sound and video are much better than the normal stream in &fmt=18, but there are some videos that look and/or sound better in &fmt=6. And there's about one or two videos that work best when compressed to standard Youtube size. :P

Oh, anyone know a youtube downloader that will download the url in it's HD quality, mine doesn't seem to support the whole fmt=18 format when I download the url, still get the same size and quality.

Depends on whether it's compressing it after. If it is, then you just go back to square one. :P
