-I don't.
-I study until I know that I will get a good grade, no time limit.
-Depends on how hard it's supposed to be. I at least try my best to do everything and recheck it afterwards.
Sure, they're useful to me. I'm not exactly sure if you are similar to me in study habits, but who knows? Maybe it'll work out!
Pretty close to my study habits, I always try and put in 110% in everything I do, and Ive studied for almost 4 hours for a final last year, Im a patient guy and Ill keep studying until I understand completely.

Though studies differ from university / college etc, I gotta say I actually put the normal amount of work into my stuff which would be as following, answering your questions:
1) Listen to music using headphones, listening to songs without lyrics. Lyrics distract you while writing / reading and well... the music is just there to shut other noises out.
2) For a test it depends. If it is just a short test of what I learned during the course so far... I'd say over the course of a week before the session, about a total of 5-7 hours. If it is an exam (1 1/2 hours), about 6 hours per week for 4 weeks
3) As much as I have to. Really can't answer it with anything definite, as the amount of paperwork, texts, essays, tests etc that I have to take care of for my studies in university differs between several dozens of pages, sometimes it is enough to just read 3 pages and mark important text passages, sometimes I have to read 60 pages of an analysis and finish a 10 -20 pages homework, analysis of my own and theory regarding the primary literature.

wat i do is disconnect everything that has gaming or internet abillity from its power source and then hid the power source and cram my arse off with text books past exam papers and print outs

I don't usually do my homework and I don't usually study on quizzes and exams but when I feel that I'm going to fail a subject I study hard but I only study by myself and don't ask for any help so its really hard
