I don't know why I feel bad for the girl leaving her uncle, but well anyway that how it is. Orie apparently feels some guilt or regret, for reasons I don't
fully understand. Maybe she just feels bad about leaving her
uncle behind to die alone.

and Orie didn't know her uncle was dying until the very end, after she had
already decided to run away with Nakakawa.
Upon further reflection, I think the last panel is meant to show us that, faced
with her uncle's imminent demise, Orie has forgiven him wholeheartedly, so
she suddenly and understandably "feels horrible" about leaving him behind
to die alone. Maybe she decides to return in order to care for him.
