Well... Of course it was in his own style, he planned his own death... Everything he's done since he and Suzaku teamed up was for that single moment.

C.C. has never died. She's essentially only been knocked out. If you'll notice, the bullet to the head she took, she did not lie there as a corpse for a bit, she took a headshot, then gave Lulu his Geass, and we promptly forgot to kick the body to see if she was dead.
When she was shot in the heart in R2, I don't believe she even deigned to fall. Simply brushed some of the blood off and gave Lulu his Geass.

Lelouch has no power in the world of C. Even C.C. and Charles can only affect the world of C to a small extent. If you re-watch the episode, you'll notice that Lelouch asked the "Gods" to stop Charles, as he had no power that could defeat Charles on his own. The "Gods" simply answered his request. As for your theory that the world mushed all those without codes, that would be false. As up until his hands disappeared, Charles had V.V.'s code. And Marianne did not have a body, what we saw in the world of C was in probably the best way I can put it, an astral projection of her.
Not an astral projection of her. It was her mind, given form by the realm of the mind, the world of C. Lulu did not ask the Gods. He commanded them, with a Level 3 Geass, to "bring about the future". Then we had a time skip. No one can ratify nor deny any sort of afterwords plotting done in C.
As for Charles, remember that Geass and the Code both are gifts of C. When C is busy digesting you, it matters not if you have the code or no. C may simply have invalidated it, in direct compliance with Lulu's "Give us the future" command.

Suzaku never had his own Geass during the incident in the WoC with Charles. So he couldn't have gotten a code from C.C., unless the world of C.C. saw it fit to give him a code as well, which theoretically would not have activated until he died, meaning that Lelouch's Geass would still be in effect. Also, just another thing that everyone either forgot completely, or forgot to mention, was the fact that C.C. sealed her code, which prevented Charles from gaining it, and also caused her amnesia(or she just sealed herself up in her mind) until Anya/Marianne appeared infront of her. I believe the reason why we haven't seen her code towards the end is that she never unsealed her code.
No one has denied the effective use of Live on! by Suzaku. And being granted a Code is not the same as being subjected to Ora...fuck, Jeremiah's Geass Canceler. He would not have made a contract for a Geass, but that does not mean C could not have been ordered to grant him a mark.
As for C.C., she sealed her code within her personal C, the world within her mind. When she had her chains broken, so too did her Code Re-emerge.
