I've watched the episodes from when Marianne awoken C.C., to when Lelouch asked/wish(Honestly, aren't they both the same thing in the end?) the "Gods"/collective consciousness to keep time going forward, and her Code never un-sealed, or was shown un-sealed. As far as I'm concerned, unless other wise shown, her code is still sealed.
Also, V.V. didn't want to die, he wanted to make a world where lies don't exist anymore along with Charles. Of course this lead him to become probably the biggest hypocrite in the series.

Marianne and Charles were mushed because their plan stood in the way of the future. When the "gods" were commanded to bring about the future, they wiped out Charles' and Marianne's existence as being in the way of the future.
Again, he never commanded the "Gods"/Collective Consciousness to banish Charles and Marianne, watch the episode again, he wished/asked for time to continue to move forward; but as you pointed already, it they were removed because they stood in the way of this wish/request from Lelouch. Now, if you are referring to the end where Lelouch told Charles and Marianne to "BEGONE!", they were already pretty much nothing but an upper torso. This scene, by the way, towards the end of Episode 21.

Well, you kind of have to know where I'm coming from. Season 1 was fine up until the last 3 episodes, because the whole massacre thing seemed really forced. I couldn't stomach what little of season 2 I saw, but I watched the last episode and it literally knocked me off my feet (seriously. The last time that happened was The Dark Knight). Since Lelouch's death is an integral part of the awesome, I feel it ruins it if he lived.
Now, the last three episodes of season 1 of Code Geass, they were sorta weird, good, but still weird with how they went. And of course the massacre was forced and out of place because Lelouch meant the command as a joke, but since Lelouch's geass leveled up, it was something he couldn't predict/know about. He understood his Geass, but he didn't understand how the Geass system works, and of course C.C. kept quiet about the whole system>.< Season 2 mirrored a lot of parts of Season 1, which in some ways was really lazy and made it look some easy to follow the early steps of it. But Season 2 was really great still, and had a really great plot line in the end.
I somewhat agree with you on the fact that for the viewer at least, Lelouch's "death" would take away a good bit of the complete awesomeness of the ending. But, it doesn't completely ruin it if he popped back up alive somewhere, as the truth is, he could have had the code since episode 21.

You're probably thinking of Darshu (otherwise known as the infamous Dark Schnieder) from the anime show known as "Bastard".

Apparently you don't know Lelouch>.> There is only one person he would have ever died for, and that would have been Nunnally. And, that is probably a good reason why he planned out his death.

Well, technically speaking, there are some people who DO have their hearts right in the middle of their chests, and than there are those who have it where their organs are on the complete opposite side of normal humans. Lelouch COULD have been one of the people with his heart in the middle, but more likely than not, he's probably like every other person. Still, where the sword pierced, if you know a little bit about the human body, would be around a major artery, and that sword was pretty freaking big, so more than not, it hit the major artery, or at least nicked it. And to prove my point, did you see all that blood? It defiantly hit/nicked the major artery in that area. It can take a few minutes or hours for a person to die from a stab wound depending on the wound and how fast the person is treated. But if a person gets a major artery cut/hit, it only takes a few seconds to a few minutes for the person to die of blood loss. Lelouch took at most a minute and half to die, so it definatly hit a major artery.
So for everyone's information, Lelouch DID DIE! But, depending on whether or not he had a code, he could have come back to life. And yes, a person with a code DOES die. They just don't stay down:p
