Most Disappointing Adaption

Seikon no Qwaser. The manga is awesome IMHO, but the anime is one of the worst adaptations I've ever seen =_= (ecchi alone doesn't make for a good show).

Claymore only because of the last 2 episodes, the prior episodes were great. It followed up to volume 11 of the manga and then they made an original ending for the last 2 episodes when the manga hasn't even ended yet closing down any possibility of a second season.

Umisho, I loved the manga but i thought the animation on the anime was terrible so i didnt watch.
To Love Ru, the manga didnt make much sense in the first place but when i watched the anime i felt like everything was thrown all over the place and that some characters didnt even get introduced and just instantly appeared in the anime.

Oppai Overlord
The last few episodes of Gantz. That was the worst anime ending ever. I can't believe they did something horrible like that.
And the whole Season 2 of Rosario Vampire was pretty bad.

Looking back, there are many that I would consider disappointments. Some I can't be harsh on simply because without them I wouldn't have dug into anime and manga as much as I have.
I will note Black Cat as being highly disappointing to me though.
Then the original Hellsing anime to an extent. Hellsing (TV) should probably have been renamed and refocused more on the character understanding the nature of being a vampire and a string of mysteries rather than an adaptation of the Hellsing manga. That would have easily been a great series to me.
The most disappointing adaptation though is one that gets my piss to a boil. Ao no Exorcists anime starts off so well that I was stoked for many seasons of Ao no Exorcist. And then the anime stabbed itself by accident. There was still hope though, the anime could still get back on track without any detrimental effect to the overall plot. But no, the anime continued to mutilate itself with another sword and killed itself in a horrifically bloody mess with no care whatsoever for it's own death.
If there is anything else that comes to mind I will post here on them. I will note that there was actually a point where I stopped watching anime altogether and never went back until around the time P&Sw/G and High School of the Dead were airing. Although prior I did watch a bit of Bakemonogatari and dropped it for a while before going back.
Further time frame, I stopped watching anime around episode 189 (or my logs say. I think I dropped it around 169 or so and messed with my old notepad logs on episodes) of Bleach. Dropped anime altogether and read only manga up til said certain point.
EDIT: Was looking at MyAnimeList to fix Bleach on there and spotted one more anime adaptation.

Itsuka Tenma no Kuro Usagi. For someone who watches the series at first, the first few episodes will get to you until you realize that alot of stuff is missing and towards the end: Fillers, God, the fillers! The story in of itself is so good though, the whole spiritual concept and the prophecy and stories of lost and unrequited love.
Had SHAFT put a little effort into it, ITNKU could have been a good series.

Higurashi. Not only did it make a bunch of unnecessary changes which made characters look stupid (Such as Mion becoming a whiny brat instead of a disciplined Yakuza-princess), but it changed the message of the story completely!
Even worse is that they watered down the story COMPLETELY into something as cliche as "Friendship trumps all!"
That was NOT what Higurashi was about (Actually its theme is about sacrifice), and I think they completely killed it.
The completely changed ending for the anime highlights just how drastic the changes were, and how watered down it was made to fit a "mainstream" audience.
