It is slightly both. She was acting irresponsible and drank too much which caused her to fall overboard. However, when someone comes onto a cruise ship, it is the crew's responsibility to make sure the passengers are taken care of and should have done better job to search for her in the water.
I understand where your coming from, and if they had done things differently it could have changed the outcome. But sadly the cruise is a business, and they have a set of rules to abide by. The bartenders were just doing there job when they were telling her to have another drink. She could've said "no", but she didn't.
If the ship's employee's paid more attention to her, they could've prevented her fall. But they were dealing with the other couple of hundred people on that same ship. Unfortunately they can't pay attention to everyone. It just isn't possible.
The Captain could have turned the ship around sooner to save her, but he has to follow procedure. So he checked the ship top to bottom to make a 100% sure she wasn't on board before he turns the ship around. He has to do this in case some one sues him or the company.

I'm sorry but I can't agree with this. Unless someone personally saw it happen, they can't be responsible nor should they be. And with the noise inside the ship plus the wind and waves outside, how could anyone hear her fall?

Like her friends for instance If they had been paying attention to a DRUNK friend in the room, and saw her leave to the balcony and not return, it would have been simple to say that "hey, I think she fell overboard."
If any passenger could have helped, it would have been her friend. But from what I've read she was in the bathroom when it happened. And when she did find out she went to the captain to plead to turn the ship around. So it seems like she did all that she could have done.
