Since the last page refers to the "mark of the hero", it would seem like the mark represents some sort of fertilisation. Along those lines I would then guess the dark mark represents a corrupt fertilisation, so the difference can be explained that way.

All of the women were fertilized. They were impregnated on the trip to the capital. My guess is the pregnancy is put in stasis until enough magical power is collected into the body to release the spell. Mind broken women no longer control their power so they "show" faster. That is the only explanation I can come up with to explain why some women were obviously pregnant and others weren't.
